In Vitro Fertilization (IVF) with double donation

IVF using a double donation of gametes represents a significant advancement for couples whose infertility stems from the inability to use their own eggs and sperm. This medically assisted reproduction procedure offers them the opportunity to start a family and experience the joys of pregnancy and parenthood.

When is IVF treatment with double donation appropriate?

IVF with double donation can be a suitable solution for couples who, for various reasons, are unable to conceive using their own gametes.
This technique may be considered in the following situations:

  • The woman has a low ovarian reserve or poor egg quality.
  • The woman suffers from premature ovarian failure or has no ovaries.
  • There is a risk of transmitting a genetic disease to the child.
  • Repeated attempts to conceive using the couple’s own gametes have failed.
  • The couple prefers double donation over the use of a single gamete donation (eggs or sperm).
  • The man has severe oligozoospermia or poor sperm quality, and the woman meets one of the above criteria.

IVF with double donation: two options at the Sanus clinic.

Option 1: Treatment with vitrified embryo transfer

  • Embryo transfer is carried out using vitrified embryos from a cryobank.

  • The selection or identification of the egg or sperm donor is not possible.
  • We carefully select donors whose phenotype matches yours, thereby increasing the chances of success.
  • The success rate is statistically lower than that achieved with double donation using fresh embryos.
  • This option is more cost-effective than Option 2.
  • Two high-quality blastocysts are transferred into your uterus.
  • Only one visit to the clinic is required – for the embryo transfer.

Option 2: Treatment with fresh embryo transfer

In the Czech Republic, the law guarantees the anonymity of egg and sperm donors. This means that recipients cannot obtain any identifying information about the donors, and vice versa. However, you do have the option to select certain non-identifying physical characteristics of the donors, such as height, weight, eye and hair colour, age, and level of education.

What are the requirements for becoming a donor at the Sanus clinic?

  • The required age for egg donors is between 18 and 32, and for sperm donors, between 18 and 35.
Female donors must already be the mother of a healthy child.
  • Donors must be in excellent health, have no genetic disorders, and test negative for sexually transmitted infections (STIs). In addition, donors must not suffer from severe mental health conditions or be regular users of alcohol or drugs.
  • In the Czech Republic, donation is strictly anonymous, ensuring the protection of the identities of the donor, the egg or sperm donor, and the recipient.

IVF with double donation: Treatment process

Preparation for treatment and selection of the donated embryo

This phase is aimed at optimising the preparation of your uterus for embryo implantation:

  1. Consultation and Diagnosis:
The first step involves a thorough consultation with a fertility specialist. During this meeting, your medical history will be reviewed in detail, and additional tests may be prescribed. These examinations are designed to assess your overall health and ensure that you are in the best possible condition for the upcoming procedure.
  2. Safety and Quality Assurance:
To ensure the safety and success of the process, all egg and sperm donors undergo rigorous medical, genetic, and psychological testing. These comprehensive assessments confirm their good health and eliminate any risk of transmitting genetic or infectious diseases.
  3. Hormonal Treatment:
Preparing the uterus for embryo implantation is a crucial step. A specific hormonal treatment is administered to optimise the receptivity of the endometrium, the uterine lining. This treatment aims to synchronise the uterine cycle with the embryo's development, thereby supporting successful implantation.
  4. Selection of the Highest-Quality Embryo:
Once the uterus is ready, the most suitable embryo is selected from among the donated ones. This careful selection takes into account your personal characteristics (hair and eye colour, complexion, height, blood group) and specific preferences to ensure optimal compatibility.
The doctor will choose the embryo with the best chances of development and implantation, based on an extensive database of donors.
Embryo Transfer

As soon as the most suitable embryo has been selected and your uterus is ready to receive it, the embryo transfer process can begin.

  • Embryo Preparation:
Cryopreservation (freezing) and thawing are generally well tolerated by embryos, although some may be more fragile than others.
  • Embryo Transfer:
The transfer is carried out with great care, using a thin plastic catheter to gently place the embryo into the uterine cavity.
  • Medical Recommendation:
To reduce the risk of multiple pregnancies, the medical recommendation is usually to transfer a single embryo.

After embryo transfer and confirmation of pregnancy

After the embryo transfer, you must continue taking the prescribed medication until it is time to carry out a pregnancy test.

This period, known as the luteal phase, is crucial for the successful implantation of the embryo. It is supported by progesterone, which helps maintain the uterine lining.

Although a home pregnancy test can be taken sixteen days after the start of the luteal phase (i.e. the beginning of progesterone), we recommend a blood test fourteen days after the embryo transfer, as it is more reliable.

If the initial HCG level is positive, a second blood test (HCG measurement) will be carried out to confirm the pregnancy.
