Consent to the Processing of Personal Data

The data subject grants consent to the processing of their personal data to the extent of the data provided (hereinafter referred to as “data”) and to its inclusion in the database of FutureLife a.s., with its registered office at Na Příkopě 859/22, Nové Město, 110 00 Prague 1, Company ID No.: 02625067, registered in the Commercial Register maintained by the Municipal Court in Prague, Section B, Insert 19615. FutureLife a.s. is a member of the SynBiol, a.s. group, managed by SynBiol, a.s., with its registered office at Pyšelská 2327/2, Prague 4, 149 00, Company ID No.: 26014343, registered in the Commercial Register maintained by the Municipal Court in Prague, Section B, Insert 14742, in the capacity of data controller, and, if applicable, to processing by a processor. This includes the offering of products or services, the provision of marketing information, and commercial communications via electronic means in accordance with Act No. 480/2004 Coll., on Certain Information Society Services and on Amendments to Certain Acts, as amended, until such consent is revoked.

The data subject acknowledges that they are entitled to the rights guaranteed by Act No. 101/2000 Coll., on the Protection of Personal Data, as amended, in particular the provisions of §9(a), §11, §12, and §21. These rights include, in particular, the fact that the provision of data is voluntary, that consent may be withdrawn at any time free of charge by contacting the data controller, and that they have the right to access personal data, the right to rectify such data, to block incorrect data, to delete it, etc. In the event of any doubts regarding the observance of their rights, the data subject may contact the controller or directly the Office for Personal Data Protection.

Personal data processed: first name, surname, age, email address. The personal data is collected via an application or electronic form through which the data subject submits the data to the controller. The personal data will be processed as described above in the controller’s electronic database, possibly by a processor with whom the controller has entered into a relevant agreement. All data is stored in digital form.

Upon request by the data subject, the controller or, where applicable, the processor is obliged to provide, without undue delay, information about the processing of their personal data. The controller is entitled to request a reasonable fee for the information provided, which shall not exceed the necessary costs associated with providing this information. Any data subject who finds or believes that their personal data is being processed contrary to the protection of their private or personal life or contrary to the law—particularly if the data is inaccurate with regard to the purpose of processing—may request an explanation or demand that the controller or processor remedy the situation (by blocking, correcting, supplementing or deleting the personal data). The data subject also has the right to submit a reasoned request to the controller, and if this is not complied with, they may contact the Office for Personal Data Protection, although they may also approach the Office directly.

The data subject may withdraw this consent at any time.
